tebIQ: the AI medical scribe for emergency medicine

tebIQ is a free-to-use AI medical scribe and clinical documentation app for emergency physicians, built by a practicing ER doctor. It turns encounters into structured ER notes with time-stamped reassessments and consults, automatic clinical scores, critical care documentation that waits for your time, and prompts that point out where more MDM can be documented when the data supports it.

See multiple patients on one recording, add addendums, create sign-out summaries for the incoming physician, and produce discharge instructions (the after visit summary) in the patient's language. Import your own dot phrases and templates for SOAP notes or ER notes, use on-device speech-to-text for medical dictation with or without AI, or add Premium AI notes. tebRounds for admissions, daily notes and one-click discharge summaries is coming soon.

Every encounter
starts as noise.

tebIQ turns it
into structure.

Not just a transcript.

An augmented clinical note.

HPI · Exam · Results · MDM · Reassessments · Disposition — each one a layer you can review. Every reassessment and consult is time-stamped.

Every AI-drafted segment is labeled, and nothing is final until you review and sign.Illustrative example — synthetic data.

Ask tebIQ about the patient it already knows.

A frontier model gives you the best answer while AI expert analysis builds alongside your note — at the same moment it is produced.Encrypted in transit and at rest (AES). Never used for training. Never sold.

One recording. Every patient. Every note.

Multiple patients on one recording get multiple notes. Add multiple addendums to the same recording. Automatic clinical scores, calculated for you.

Say “Discharge to home.” In their language.

The moment you say it, tebIQ understands the patient speaks another language and produces the discharge summary in that language automatically.Illustrative translations shown.

tebRounds, built in.

Admit patients to ED Obs, write the daily notes, and finish with a one-click discharge summary in about 30 seconds. Coming soon, for hospitalists, surgeons, OB-GYN and every specialty that sees patients daily.

Sign-out, handled.

Select multiple patients and choose sign-out for a summary to hand the incoming physician. Record a colleague giving you sign-out for a patient-by-patient statement of facts and pending studies. Record your PA, NP or resident and get an instant statement that you agreed with and took part in the plan of care.

Critical care, filled in for you.

When the patient meets critical care criteria the fields fill automatically, and you can remove them instantly if you disagree. Nothing copies until you enter a time.

Built for complete billing.

The note prompts you and points out segments where more of your MDM can be documented, when the relevant data is there.

Free to use. AI when you want it.

Import your own dot phrases and use tebIQ as your note-writing station. Use the app's on-device speech-to-text for a bare transcription, then write your note with our templates or your own, with or without AI. Want AI? Get 20 Premium AI notes to see the difference.

It's 2:47 a.m. I've made every decision these patients needed — and I still have eleven notes left to write. So I'd stay late, or take the charts home and finish them on the couch while my family slept. The documentation always won.

The AI scribes I tried transcribed beautifully, but an emergency note isn't a transcript — it's an argument: the medical decision-making that justifies why I ruled out the killer, why this one goes home and that one stays. They were built by people who've never written a discharge at 3 a.m. So I built the one I actually wanted to use on shift — between patients, fixing what annoyed me that night. Built on shift, not in a boardroom.

I'm still on the schedule, and I read every message. When a pilot physician flagged duplicate charts one evening, we found the root cause and shipped the fix in the next build within 24 hours. That's the whole point: the only AI scribe written by the doctor using it.

Questions people ask

What is an AI medical scribe?

An AI medical scribe turns a patient encounter into a draft clinical note, so you spend less of your shift typing. tebIQ is an AI medical scribe built by a practicing emergency physician: you capture the encounter, tebIQ drafts the note, and you review and edit every word.

Does tebIQ write SOAP notes?

tebIQ drafts structured clinical notes: history, medical decision making, assessment and plan. You can import your own templates and dot phrases, so a SOAP note can be laid out the way your group writes it.

Is tebIQ an ambient scribe or a dictation app?

Both ways of working fit. You can record the encounter or dictate the note afterward, ambient or dictated, your choice, and tebIQ drafts the note either way for you to review.

Does it work for nurses and urgent care?

Urgent care, yes: there is an urgent care workflow with fast notes and discharge instructions in 14 languages. tebIQ is built and tested by a physician for physician notes, so nurse notes and nursing documentation are not what it was designed for today.

Is my patients' data used for training?

Encrypted in transit and at rest (AES). Never used for training. Never sold.

How much does an AI medical scribe cost?

tebIQ is free to start: 20 free AI notes, then 10 free each month, and residents are free. Paid plans are per clinician; the current plans are on the pricing page.

Your name is not for sale. Not to advertisers. Not to data brokers. Not to drug companies. Not ever.

Your name, your practice address, your phone number, your email, your specialty, and the fact that you use tebIQ at all — none of it is sold, rented, traded, or handed to anyone building a physician target list. There are no ads in this product and there will not be. This one has no exceptions.

The only outside companies that ever touch your data are the vendors that run the service: speech-to-text, an AI model provider, and cloud hosting. They work for tebIQ under contract, and they handle your data only to run the service.
