AI medical scribe for emergency medicine: ER notes, MDM-based E/M leveling, critical care time, discharge instructions in 14 languages.
Emergency Medicine
tebIQ is an AI medical scribe for emergency medicine, built by a practicing emergency physician. It drafts the note while it levels the chart on medical decision making, captures critical-care time, and runs sepsis screens and validated scores. Your data is encrypted in transit and at rest (AES), never used for training, and never sold.
tebIQ is decision support for the treating clinician. Every level, score, screen and draft is a suggestion to verify against the patient in front of you; you decide and you sign.
What the ER workflow does
An emergency department chart is a record of decisions made under time pressure. tebIQ captures the encounter and drafts the structured note — HPI, medical decision making, and assessment and plan — and gives you the verbatim skeleton immediately, so the AI-structured sections are there when you want them rather than standing between you and the chart.
The note is a draft for the treating clinician. You read it, change it, and sign it.
MDM and E/M leveling
tebIQ levels each chart on MDM density with deterministic rules, so the suggested level is the one the documentation supports. Before you sign, a billing-gap review lists documentation that is missing for the level you are aiming at, so the gap is closed while the patient is still fresh in your mind and not in a later query.
Critical-care time
When a patient needed critical care, tebIQ proposes the time attestation and you confirm it. Nothing is attested without your confirmation: it proposes, then you confirm, which is how the CMS-compliant attestation is meant to work.
Sepsis, scores and can't-miss diagnoses
tebIQ is decision support for the treating clinician. Every level, score, screen and draft is a suggestion to verify against the patient in front of you; you decide and you sign.
- Sepsis screening — SIRS, qSOFA and SOFA — runs on every chart in real time.
- Validated clinical scores, including PERC, HEART and NIHSS, are computed live at the point of care.
- Critical rule-outs surface the can't-miss diagnosis before disposition.
Many patients at once
Multi-patient session handling keeps several open charts apart, and structured handoffs let the next physician see where each patient stands without a verbal retelling of the whole chart.
Discharge handouts and the interpreter
Discharge instructions are drafted for you and can be produced in 14 languages. The 14-language handouts are machine-translated from the English text, so read the English you are handing out as you would any handout. In the mobile app, an interpreter for 12 patient languages runs fully on the device.
tebDictate, our free phone-to-computer dictation tool, needs no account and is a way to try the microphone before you try anything else.

Frequently asked questions
Is there an AI scribe built for emergency medicine?
Yes. tebIQ was built on shift by a practicing emergency physician. It drafts the note and levels the chart on medical decision making, captures critical-care time, and runs sepsis screens and validated scores while you work.
How does tebIQ decide the E/M level?
It levels the chart on MDM density using deterministic rules, and a billing-gap review lists missing documentation before you sign. The level is a suggestion; you decide and sign.
Does tebIQ capture critical-care time?
Yes. tebIQ proposes the critical-care time attestation and you confirm it. Nothing is attested until you confirm.
Which sepsis screens and scores does it run?
Sepsis screening with SIRS, qSOFA and SOFA runs on every chart, and validated scores including PERC, HEART and NIHSS are computed at the point of care. They are decision support, not a diagnosis.
Can it produce discharge handouts for non-English-speaking patients?
Yes. Discharge handouts can be produced in 14 languages. They are machine-translated from the English text. The mobile app also has an on-device interpreter for 12 patient languages.
Can I handle several patients in one shift?
Yes. tebIQ supports multi-patient sessions with structured handoffs, so each patient's chart stays separate.
Is my data used to train AI models?
No. Encrypted in transit and at rest (AES). Never used for training. Never sold. See the Data Pledge and the Security page for more.
What does tebIQ cost?
You can start free with 20 AI notes, then 10 per month. Paid plans are listed on the pricing page.
